CalvertHealth, the sole community hospital in Calvert County, Maryland, sought better access to real-time health data and stronger community engagement, especially during the COVID-19 pandemic. Partnering with Conduent’s Healthy Communities Institute, the hospital adopted an integrated platform offering dashboards, reporting, and consulting services. This enabled continuous tracking of health priorities like cancer, heart disease, mental health, and nutrition while uncovering critical issues such as food insecurity. The platform drove a 14-fold increase in survey responses, guided strategic interventions like a drive-through farmers market, and strengthened data-driven decision-making for long-term health improvement.
