Progress has been made in integrated, patient-focused models—especially at state levels—but lacks national coordination. Patient-reported measures are growing, though inconsistently applied across regions and settings. Funding remains the weakest area, with most care still reimbursed via fee-for-service. Pilot pay-for-performance and bundled payment initiatives are underway, but scale is limited. Barriers include data sharing gaps, unclear metrics, and provider resistance. A coordinated national approach is needed to fully realize VBHC benefits.
