The COVID-19 pandemic highlighted deep health disparities and the need for health systems to act on social determinants of health. Renown Health in Nevada partnered with Conduent’s Healthy Communities Institute to move from commitment to measurable impact, guided by six elements: open leadership, reliable shared data, thoughtful community needs assessments, neutral experts, accountable implementation plans, and collaborative partnerships. Using the HCI Platform, Renown centralized data, identified 13 priority health concerns, and developed targeted interventions. This partnership fostered community ownership, improved equity strategies, and strengthened long-term public health outcomes.
