Diabetes Free South Carolina, funded by the BlueCross BlueShield of South Carolina Foundation, set out to reduce disparities and improve outcomes for diabetes patients through prevention, education, and care. With more than 13 percent of adults affected and high rates of prediabetes, DFSC needed a modern, secure, and independent data platform to replace legacy systems.
